Auto-Pay/Credit Card Payment
All auto-pay and payments made over the phone will have a $3 service fee added onto their total.  As a friendly reminder, we do not accept any charges under $20 on credit AND/OR debit cards.  Please set printer to print on both sides.

Open Gym Waiver 
Open Gym/Open Play waivers must be completed by a parent or legal guardian.  Waivers will be kept on file for the 2022-2023 (September – May) season.  New waivers must be filled out at the start of each season.

Trial Class Form & Waiver
Trial Classes are for brand new students/families to our program.  You are only allowed ONE trial class per child.

Private Lesson Waiver
This waiver is meant for those students who are only taking private lessons at the gym and not enrolling in any classes.  If your student is enrolled in a class or planning to enroll during the 2022-2023 (September – May) season,  you will fill out the registration form, which includes the waiver.

Withdrawal Form

While tuition is due monthly, we do not consider each month a new session.  During the school year (September-May), your child’s enrollment is carried over each month.  Should you choose to withdraw your child from our program, a two-week written notice is required, as stated on the registration form signed at the time of enrollment.   If a two-week notice is not provided, we will assume your child is still enrolled and we will continue to add tuition charges and late fees on your account, for which you will be held responsible.  Please fill out and either email this form or drop off at the front desk so the office staff can update your account.
